Step-by-step instructions to help you manage your account and complete the most common account actions.
What do you want to do?
Update contact information
On the portal
2. Navigate to your name in the upper right-hand corner.
3. Select ‘My Profile.’
4. Select ‘Manage Contact Information’ from the left-hand menu.
5. Add new contact information by clicking the ‘Add’ button, or edit existing contact information by clicking the small green ‘Edit’ button underneath your address/phone number on record.
Via form
1. Download our Change/Update Contact Information Form.
2. Complete and sign the form.
3. Send form to Provident Trust Group by:
- Email at newaccounts@trustprovident.com
- Fax at (702) 253-7565, or
- Mail to 8880 W Sunset Rd #250, Las Vegas, NV 89148
Initiate a cash transfer
On the portal
2. Navigate to Accounts > Transfer Funds on the left-hand menu.
3. Select the blue “Create New+” button in the top right-hand corner. This will walk you through entering the information needed to complete the Incoming Transfer Form.
4. Once your resigning custodian’s information is entered, select either Option A) Complete Transfer or Option B) Partial Transfer.
5. Enter the amount of cash you would like to transfer on the line beside the “Cash” option.
NOTE: If selecting a “Complete Transfer,” please note that Provident Trust Group cannot hold publicly traded stocks, bonds, or mutual funds. This means that all assets must be liquidated prior to submitting the request.
6. If your resigning custodian accepts transfer requests via fax, you can complete the form using the digital signature option and submit to Provident Trust Group. We will review your form and fax it to your current custodian with our Letter of Acceptance. If your resigning custodian requires original signatures on documents, you will need to print the form, sign it and mail it to our offices for processing.
Via form
1. Complete the Incoming Transfer Request Form.
2. Select either Option A) Complete Transfer or Option B) Partial Transfer
3. Select “Cash” and enter the amount of cash you would like to transfer on the line beside the “Cash” option.
NOTE: If selecting a “Complete Transfer”, please note that Provident Trust Group cannot hold publicly traded stock, bonds, or mutual funds. This means that all assets must be liquidated prior to submitting the request.
4. Send your Incoming Transfer Form to Provident Trust Group so that we can execute the document and include a Letter of Acceptance with our submission to your current custodian. This form can be sent to Provident Trust Group via fax or email (if the resigning custodian accepts faxed transfer requests) or mail (required if the resigning custodian requires original transfer requests).
NOTE: Do not send the Incoming Transfer Form directly to your resigning custodian. Sending this form directly to them without our signature or letter of acceptance will result in the rejection of your transfer request.
Initiate an asset transfer
On the portal
2. Navigate to Accounts > Transfer Funds on the left-hand menu.
3. Select the blue “Create New+” button in the top right-hand corner. This will walk you through entering the information needed to complete the Incoming Transfer Form.
4. Once your resigning custodian’s information is entered, select either Option A) Complete Transfer or Option B) Partial Transfer.
5. Put a description of the asset you are transferring in the asset description box.
6. You will be required to upload a recent statement from your current custodian. Please review the Incoming Asset One Sheet for additional information about the documents required to transfer assets to your account at Provident Trust Group.
7. If your resigning custodian accepts transfer requests via fax, you can complete the form using the digital signature option and submit to Provident Trust Group. We will review your form and fax it to your current custodian with our Letter of Acceptance. If your resigning custodian requires original signatures on documents, you will need to print the form, sign it and mail it to our offices for processing.

3. Select “Asset” and in the box beside the Asset line, put a description of the asset you are transferring.
NOTE: Requests to transfer an asset to Provident Trust Group must come with a recent statement from your current custodian. Please review the Incoming Asset One Sheet for additional information about the documents required to transfer assets to your account at Provident Trust Group.
4. Send your Incoming Transfer Form to Provident Trust Group so that we can execute the document and include a Letter of Acceptance with our submission to your current custodian. This form can be sent to Provident Trust Group via fax or email (if the resigning custodian accepts faxed transfer requests) or mail (required if the resigning custodian requires original transfer requests).

Initiate a rollover
Via form
Rollover transactions will begin with a distribution by the resigning custodian or plan administrator.
To complete a rollover:
1. Contact your resigning custodian or plan administrator to ensure your account is ready
to roll over (i.e., investments have been liquidated, if required).
2. Confirm the resigning custodian’s requirements. For example, do they have their own form that needs to be completed? Can a rollover be completed via the phone?)
3. Provide the resigning custodian or plan administrator with the Deposit Instructions Form if they are sending Provident funds.
4. Complete and remit the Deposit Submission Form to Provident.
5. Once your funds arrive at Provident, you will receive an email confirmation that the deposit was received. You can also view your cash balance in your portal.
Please note that Provident cannot influence the resigning custodian or plan administrator to process a rollover request more quickly. If you are concerned about the turnaround time of your request, contact your resigning custodian approximately seven days after the initial request submission date.
Transfer funds out of Provident Trust Group to a new custodian
Via form
Transfers of funds out to a new custodian start with your new custodian. You should review that custodian’s requirements for transfers in and they will submit appropriate paperwork to Provident Trust Group.
Transfer outs can take up to five to seven business days depending on the transfer request.
NOTE: Provident Trust Group requires a Medallion Signature Guarantee on all transfers submitted by fax or email. All transfer out requests should be received with a Corporate Resolution, Delivery Instructions, and a Letter of Acceptance. If the Transfer Out request is for assets in kind, we will require the letter of acceptance to specifically state that the new custodian will accept the asset being transferred to them as many traditional custodians do not accept alternative assets.
Provident Trust Group only requires a medallion signature guarantee on outgoing transfer requests that are faxed or emailed to us. If the originals are sent by mail, we do not require a medallion signature guarantee.
Update your beneficiaries
On the portal
2. Navigate to Accounts > Beneficiaries on the left-hand menu.
3. Select the blue Update Beneficiaries button and enter your revised beneficiary information.
4. You will receive an email confirmation once you save your updated beneficiary information.
Via form
1. Visit the CHANGE/UPDATE ACCOUNT INFORMATION section of our Forms page.
2. Locate the forms beginning with “Beneficiary Designation -” and select the version that matches your account type.
3. Complete, sign, and return the form to Provident Trust Group via mail, email, or fax (information listed at the bottom of the form).
